The Extended Care team consists of physicians, nurse practitioners, nurses,
physical, occupational and speech therapists, dieticians, social services,
case managers and onsite pharmacists who join forces to develop individualized
patient treatment plans. Case conferences are held with the patient, their
family, and their physician as part of the recovery plan. All patients
receive therapies according to their tolerance and their need. While we
believe patients at all stages of the recovery process benefit from early
and aggressive intervention and therapy, we develop a tailored plan of
care for each patient based on his or her abilities and needs.
Discharge includes discussing recuperation goals and plans with the patient's
physician. During care delivery, the plan is re-evaluated with involvement
of the patient, physician and family.
