Your Hospital Bill
Your Lake Charles Memorial Hospital bill includes charges for goods and
services rendered by the hospital, but does not include any professional
services. for example, depending on
your care, you may receive separate bills for such services as a diagnostic
physician who analyzes your hospital x-ray or lab work,consulting specialty
physicians that may have been requested by your primary care doctor based
on your treatment plan, or ambulance services that are not a part of the
Memorial care system. Since these may not be a part of the Memorial system,
should you have questions about such bills, those service providers should
be contacted directly.

Each hospital visit is billed individually, and itemized bills are available
You may contact our Patient Financial Services Department at 337-494-3265 with any questions or concerns you may have about your bill. The billing
and financial services office is located at 3050 Aster Street in Lake
Charles and is open Monday – Friday from 8:00 am to 4:30 pm.

When corresponding with the Business Office, please provide your full name,
daytime and evening telephone number, account number, and a brief summary
regarding any information you wish to provide to us or the nature of your
question. A Patient Financial Service Representative will follow-up with
you as appropriate within two business days.
Lake Charles Memorial Health System will bill your insurance(s) prior to
billing you for any balances left on your account. However, if your insurance
fails to pay the claims, you will be responsible for any and all costs
incurred with the provision of your care. All balances are expected to
be paid in full within 30 days of being billed, or you must make appropriate
Notice: Health care services may be provided to you at a network healthcare
facility by facility-based physicians who are not in your health plan.
You may be responsible for payment of all or part of the fees for those
out-of-network services, in addition to applicable amounts due for co-payments,
coinsurance, deductibles, and non-covered services. Specific information
about in-network and out-of-network facility based physicians can be found
at the website address of your health plan or by calling the customer
service telephone number of your health plan.
Please let us know of any special circumstance that may impact the timely,
accurate and complete payment for your services. Our Financial Service
Representatives are able and willing to accommodate you in any way, but
we need to know of special circumstances in advance of your service.
Memorial’s basic payment plan can be arranged after a brief interview
with one of our Patient Account Representatives. Once a payment plan has
been authorized, failure to make regular and routine payments may result
in the debt being transferred to a collection agency.
There are many programs available for the uninsured. If you are uninsured
and need assistance with your bill, please contact one of our Patient
Account Representatives at 337-494-3265 for further information.
Medicaid Application Center
Memorial Hospital does have a Medicaid Application Center within the hospital.
The office hours are 8:00 am to 4:30 pm. Applications requested are by
appointment only. Please call 337-494-4637 to set up, cancel or reschedule
Memorial is committed to extending financial assistance, and free or reduced
fee care, to qualifying individuals. If payment on your health care expenses
could create a financial hardship for you, please contact a Financial
Service Representative, who will help you apply for financial assistance.
Applications will be processed and approval will be determined based on
specific criteria. Information you provide is confidential and is only
reviewed by the staff processing your application. If approved, your financial
obligation to Memorial Hospital will be reduced. We will not discriminate
in the determination of financial eligibility on the basis of race, color,
creed, sex, age or handicap.
