Vacations
Regular full-time & regular part time employees.
After one year of employment. Begin accruing benefits immediately upon hire.

Holidays

Immediately upon hire. Floating Holiday awarded after completion of 90 day evaluative period.

Sick Leave

Health Insurance Medical/Dental

1st of month after hire or during annual enrollment period in May. Dental is effective 90 days after enrollment.

Coverage with Employee Benefit Services includes $2,000,000 per person lifetime maximum payable on the medical and dental plan.
Life Insurance
Regular full-time employees
1st of month after completion of 90 day evalutive period.

1 - 1/2 times annual salary up to a maximum of $50,000 insurance.
Long Term Disability
Regular full-time employees
1st of month after completion of 90 day evaluative period.

50 % of base salary beginning on the 91st day of disability.
Retirement
All employees who are at least 21 years of age and who worked 1000 hours during the calendar year
End of calendar year after completion of 12 months employment

Amount contributed per year based on percentage of employees gross yearly wages. 20% vesting each year beginning with the 3rd year. Fully vested after 6 years.

Employee must sign contract to work 2 years once degree earned. Must receive prior approval and complete course with a C grade or better. 100% paid for FT and 50% paid for PT. Area of study must be related to employment at LCMH.
Child Care Assistance
