FINANCIAL COUNSELING
Memorial's Financial Counseling Department works hand-in-hand with our Patient Registrars to assist our patients and their families in applying for financial medical assistance programs.
If you or a family member are a patient at Lake Charles Memorial Hospital and wish to be screened for financial medical assistance, call (337) 494-4637, Monday through Friday, 8am to 5pm.
PROGRAMS
Financial assistance programs that are available at Memorial include, but are not limited to:
- Children's Health Insurance Program (LaCHIP)
- Medicaid
- Indigent Program
- Crime Victims
- CMS 1011
- Financial Discount Program
- Out of State Medicaid Programs
- Social Security Disability
- QMB Qualified Medicare Beneficiary Medicaid
- Veterans (VA) / Millennium Bill
PAYMENT OPTIONS
When meeting with a financial counselor, please be sure to bring proof of the following:
- Identification - Valid photo ID card or driver's license.
- Birth Certificate - please bring with you for proof of Citizenship.
- Income - for the last three (3) months, such as payroll check stubs, employer letter, award letter from Social Security Administration, Workers' Compensation documentation or, if self-employed, a ledger of expenses and income.
- Residency - Statements showing your address.
- Unemployed status (if applicable) - a current letter stating you are enrolled with State Workforce
- Development, a current statement from your physician stating your disability status (and for how long) or a letter from Social Security stating your disability.
- Shelter-In-Kind - a written statement from the person who is currently supporting you that includes the dollar amount provided for your care.
- Savings or Checking account - Current bank statements for past three (3) months for all accounts.
- Food stamps - current letter from the Department of Human Services showing certification for assistance.
- Burial or life insurance policies - bring the complete policy.
- Medical insurance - Medicare card, Medicaid card, medical insurance cards, Veterans Administration card, etc. for all household members.
- Ownership of property - home deed, automobile title(s), and land deed(s).
- SSI or other programs applied for - a letter, phone numbers, names or case number of when and where and with whom you applied.
