The Annual Fund is divided into several sponsorship levels and represents an integral and vital tradition of giving. The Annual Fund helps to promote the state of medical excellence and personalized care that make Memorial Hospital such an extraordinary community asset. Within the Annual Fund, you may make contributions of varying sizes, each designed to meet your individual financial needs and those of the hospital. Annual gifts are acknowledged within one of the following recognition “Club” levels:

The Legacy Circle *$10,000 and above
This, our highest level, honors and recognizes the generosity and vision of those who have chosen to establish a legacy of care by naming The Foundation as a beneficiary in a bequest, trust, retirement plan, life insurance policy or gift-in-kind. The Foundation honors any request for anonymity. As a Legacy Circle honoree, you may choose to make unrestricted bequests to The Foundation or designate a specific Named Gift Opportunity.

Enrollment in this honorary society is simply a matter of providing documentation to The Foundation of the creation of a legacy gift at a minimum amount of $10,000. Should a Legacy Circle donor need professional assistance from an attorney or certified public accountant, The Foundation will be happy to provide a list of authoritative professionals.

In recognition of your generosity, Legacy Circle members will be invited to attend the bi-annual “state-of-the-hospital” luncheons with Larry Graham, Memorial Hospital’s Chief Executive Officer, and members of the President’s Club. Legacy donors’ names will be engraved on a special Legacy Circle plaque, which will be displayed on the main floor of the hospital. Additionally, donors will be invited to the Foundation Recognition Banquet and recognized in the donor issue of the Memorial & You community newsletter.

The President's Club *$5,000
Couples or individuals can be recognized as members of the President’s Club by donating a minimum of $5,000. Your contribution can be pledged at equal annual contributions of at least $1,000 for up to five years by check, the transfer of stocks, or a donation of real estate valued at a comparable amount. As a member of the President’s Club, you will be invited to attend the biannual “state-of-the-hospital” luncheons with Larry Graham, Memorial Hospital’s Chief Executive Officer, and members of the Legacy Circle. Your name(s) will be engraved on a special President’s Club plaque, which will be displayed on the main floor of the hospital. Additionally, you will be invited to The Foundation’s Annual Recognition Banquet and recognized in the Donor issue of the Memorial & You community newsletter.

The Ambassador's Club *$1,000
Couples or individuals can be recognized as members of the Ambassador’s Club by donating a minimum of $1,000. Your contribution can be pledged at equal annual contributions of at least $500 for up to two years by check or the transfer of stocks. As an Ambassador Club member, you will be recognized on a special Ambassador’s Club plaque, which will be displayed on the main floor of the hospital. Additionally, you will be invited to The Foundation’s Annual Recognition Banquet and recognized in the donor issue of the Memorial & You community newsletter.

The Benefactor's Club *$500
Couples or individuals can be recognized at the Benefactor’s Club level by donating a minimum of $500. Your donation is recognized on a special Benefactor’s Club plaque, which will be displayed on the main floor of the hospital. Additionally, as a contributor at the Benefactor level, you will be recognized in the donor issue of the Memorial & You community newsletter.

The CARE Club *$100 or more
Donations to the CARE Club, “Community Assistance Reaches Everyone”, can be made in any amount and will support the areas of greatest need at Memorial Hospital. As a donor of $100 or more annually, you will be recognized in the donor issue of the Memorial & You community newsletter.

All gifts to The Foundation at Lake Charles Memorial Hospital are tax-deductible as prescribed by law. We recommend that all donors discuss major gifts with their personal attorney or certified public accountant. Appropriate consul is also available through The Foundation should you desire assistance.
