This web site will allow you to request refills of prescriptions that you have previously had filled at Lake Charles Memorial Hospital’s Medical Plaza Pharmacy. If you prefer to use the telephone, call (337) 494-2989.
Please submit refill requests 1-3 days prior to depletion of medication to allow time for processing.
If your label says 0 refills remaining or the “until” date has been reached, you will need a new prescription. The pharmacy will attempt to contact your physician when you request a prescription that has no refills. Please allow us 2-3 days to obtain a new prescription.
Medical Plaza Pharmacy is available Monday through Friday from 8am – 5pm. If you request a refill after normal business hours your prescription will be processed on the next business day. If you need your medication immediately please contact your physician.
