are not needed to transfer a patient from
your emergency department to Memorial's Emergency Department.

- Admit orders from a Memorial physician are needed to transfer a patient from your hospital inpatient admission to a Memorial inpatient admission
The House Supervisor (Phone Number: (337) 480-8430) can help:
- Facilitate the details necessary for transfer, including any paperwork and medical documents that Memorial will need
- Coordinate nursing unit and bed arrangements for the patient
Outside facilities can help ensure patient identification by faxing the patient's demographics or face sheet immediately to (337) 494-2956.
