How do I sign into the patient portal?

  1. Acceptance & Email Address: Provide your physician's office with your name, date of birth, and email address.
  2. Portal Activation: Our staff will activate your account.
  3. Welcome Email: Our system will automatically send a welcome email to your email address. The welcome email will include a link to the portal and a temporary password that will be needed for you to initially access the portal.
  4. Portal Access: Click the link in the welcome email (or use the link below) to access the login page. You will log in with your email address and temporary password for the first time.

I'm having trouble with the patient portal. Who can help me?

For questions or issues regarding your patient portal, such difficulties as logging in, please contact your physician's office directly. You can also call (337) 494-2343 or securely email helpdesk@lcmh.com. When emailing, please include your name, date of birth and a contact phone number. Note: replies will respond from svcdsknew@lcmh.com.

How can I get a question to my medical provider?

You can securely communicate with your physician office between visits for NON-URGENT issues and questions. This is notnecessarily a message sent directly to your doctor, but potentially may be screened and routed by our nurses and administrative staff. You may also call with any questions or concerns. The office staff, physician or nurse will call you back in a timely manner.
