Patient Portal

Memorial patients have 2 portals, one for hospital health records and one for Memorial Medical Group physician offices. These interactive web portals enable all patients and their families to take an active role in their care by providing easy, secure access to health information. All of the information in the patient portal comes from your Memorial electronic health record.


The Follow My Health patient portal allows you to:

  • Access your inpatient health information
  • View your laboratory and radiology results
  • Find a list of current medications and allergies
  • Update personal information and more

In addition to handling your own care, parents and healthcare proxies with authorized consent have the ability to access the records of those individuals for whom they manage care. If you are having trouble logging on to the portal, please call (337) 494-2343 or securely email helpdesk@lcmh.com. When emailing, please include your name, date of birth and a contact phone number. Note: replies will respond from svcdsknew@lcmh.com.

My MMG Health is one portal for all your MMG doctors where you can:

  • Securely communicate with us between visits for NON-URGENT issues and questions.
  • Download and securely transmit a summary of your medical record to other web-based applications and providers of care.
  • View Records of your current medications, medication allergies, and medical problems.
  • View Appointments and summaries of past visits and view details for upcoming appointments.
  • View Results along with a personal note from your doctor about the results. You will be able to acknowledge your review of the results through the portal.
  • Review Educational Materials we may post about your condition. This may be links to a website, documents to read, or even videos to watch.
