Better Heart Failure Management
The Heart Failure Clinic is a service of Memorial Medical Group Heart &
Vascular Center.
Services:
- Post discharge aid in understanding instructions
- Medication compliance, reconciliation and management
- Dietary and lifestyle changes as needed
- Tailored heart failure education for better self-management

Outpatient infusion therapy including:
- Diuretics by IV Push or infusion up to 8 hours
- Dobutamine or other infusions up to 8 hours
- IV hydration up to 8 hours
- Blood product transfusion services
- Advanced cardiac therapies such as cardiac resynchronization, left ventricular
devices, and heart transplant
American Heart Association Best Practices Guidelines:
- Guideline-directed medical therapy for heart failure on admission
- Transitional summary upon discharge to next provider
- Follow-up appointment within 7 days of discharge
Inclusion Criteria:
- Patients with a primary diagnosis of congestive heart failure and an ejection
fraction of less than or equal to 40%
- Patients with diagnosis of pulmonary hypertension
Target Interventions:
- Initiate AHA guideline recommended therapy
- Implement effective patient and family education
- Integrate palliative care and screening for advanced heart failure therapy
- Provide discharge summary to next provider at time of discharge from Health
Failure Clinic
- Provide post discharge appointment documentation
